Agreement for services provided by Horizon Digital Dental Lab.
By engaging Horizon Digital Dental Lab LLC for dental laboratory services, you agree to the following terms.
Services are limited to those described in your prescription, work order, quote, or written agreement (e.g., fabrication of crowns, bridges, implant restorations, removable appliances, and related laboratory work). Additional work, remakes, or rush services may incur extra fees as quoted or invoiced.
Fees for laboratory services are due according to the payment terms on your invoice or contract. You are responsible for shipping, taxes, and third-party charges (e.g., courier fees) unless otherwise agreed in writing.
The client (dentist or authorized practice representative) agrees to provide accurate prescriptions, adequate impressions or scan data, shade information, and timely approvals. You remain responsible for clinical decisions, try-in, and final seating of prosthetics.
Cancellation or changes to a case may be subject to fees depending on production stage and materials committed, as stated in your order terms or lab policies. Ongoing accounts may be terminated in accordance with the notice period in your agreement, if any.
